- 1). Download an application form. The department of transportation's website provides an application (Form MV-145A) online at http://www.dmv.state.pa.us/pdotforms/mv_forms. Be sure to go to this exact url. The Pennsylvania's DOT website has many dead links making it difficult to navigate.
- 2). Fill out your portion of the application. The form requires additional information from either a doctor and a police officer.
- 3). Take the form to your doctor or to a police officer to complete. Be sure the doctor you have work on the application is the same doctor that works with the disability that keeps you from driving. If you do not have a doctor to fill out the form you must have a police officer certify that a handicap placard is necessary.
- 4). Hire a notary to witness your signing of the application, and then to notarize it.
