There are limited data on the long-term follow-up of patients with AIP. As mentioned previously, up to 7% of patients with parenchymal atrophy, calcification, and exocrine and endocrine deficiencies mark the burnt-out phase of this chronic disease. Gupta et al studied the resected pancreatic specimens of patients with AIP and found that these individuals are at increased risk of PC and should be monitored closely. Another multicenter retrospective cohort study showed that in the first year after diagnosis, AIP is associated with an increased risk of PC and other cancers, with gastric cancer being the most common. A Japanese study showed that the incidence of pancreatic cancer is increased in patients with AIP and surveillance for PC should be pursued in these patients similar to the patients with chronic pancreatitis. The annual incident rate of PC in type 1 AIP was found to be 0.92%.
