Replacing a Social Security Card in Atlanta, Georgia
- 1). Go to your local Atlanta Social Security office to apply for a replacement Social Security card. Office hours are Monday through Friday between 8 a.m. and 5 p.m.

This application is found at your Atlanta Social Security Administration office as well as the SSA website; you can download and print a copy to fill out at home, which makes your appointment at the SSA office faster.
- 3). Complete form SS-5. Include your name, Social Security number, date of birth, place of birth, parents' names and your current address. Sign form SS-5.
- 4). Gather documents to prove your identity. The SSA accepts driver licenses, state-issued ID cards, military and government ID cards, passports, naturalization certificates, birth certificates, reports of birth abroad and any other legal document that shows your photo and date of birth. The SSA only accepts original or certified copies of your identification documents.
- 5). Give your identification documents and application to a clerk at the SSA office. There is no fee to apply for a replacement Social Security card. Your new card will arrive in the mail one to two weeks after your application is submitted.
