How to Apply for an Oklahoma State Tax ID
- 1). Download the new business start-up packet from Oklahoma's government website (see Reference). Print out the downloaded PDF file.
- 2). Read the instructions in the booklet carefully, as the different business types are explained. If applying as a Limited Liability Company or a corporation of any type, the form included in the booklet will require you to provide a Federal Employer Identification Number (FEIN) in the appropriate block. If applying as a sole proprietor where the business will be operated strictly by yourself, then only your Social Security Number (SSN) will be required in the appropriate block.
- 3). Fill out all information as requested on the form, and be complete while doing so. Never leave any blank empty if it doesn't apply to you. If you will be operating the business under any name other than your own personal name, be sure to enter the name of the business under the DBA (Doing Business As) section of the form.
- 4). Write a check for the current fee amount listed in the Oklahoma Sales and Use Tax application, which is $20, as of 2010, and make the check payable to Oklahoma Tax Division.
- 5). Place the completed form, along with the check, into an envelope and mail the envelope to Oklahoma Tax Commission, Taxpayer Assistance Division; Post Office Box 26920; Oklahoma City, OK 73126-0920.
