    • 1). Visit the U.S. Government Social Security website, call 800-772-1213 or visit the Social Security office in your area and request an application for a new Social Security card. Ask for Form SS-5.

    • 2). Complete form SS-5 in its entirety.

    • 3). Provide proof of your identity. The Social Security office will accept your U.S. driver's license, state-issued identification card or passport as proof of identity. If you were born outside of the United States or if the Social Security office requests proof of your citizenship, you must also provide a U.S. birth certificate, U.S. consular report, U.S. passport, Certificate of Naturalization or Certificate of Citizenship.

    • 4). Mail or deliver your application and required documents to your local Social Security office.
