Active listening.

Don't be shy to ask questions once in a while especially if you think that your clients missed crucial information.
You need to obtain as much data as possible so you can easily figure out the exact learning needs of these people.

Let me give you an example; if you are offering face-to-face mentoring, it would help if you can talk some place where these people can simply "loosen up".

As a mentor, your clients surely look up to you.
They will not sign up if they don't in the first place.
Tell them your failures and your success stories so you can inspire them.
Convince them not to repeat the same mistakes you have done in the past and take control of their life by doing the things that have helped you succeed.

Keep a journal for each of your clients.
If you are serving more than one person, it would work to your advantage if you keep a separate journal for each of your clients to simply avoid confusion.
In here, you can write down the things that you have talked for the day, their achievements, the things that they are struggling with, and the advice that you have given them.
You can review these journals before starting a new session.
