    • 1). Visit the IRS website and click on the "forms and publications" link, then choose "form and instruction number" from the list of download options. Type in "1040-X." 1040-X is one of the few forms which does not change annually, so you will not need to choose by year. Download the instructions for reference. You will need the current version of Adobe Acrobat Reader, which you can download for free.

    • 2). Order form 1040-X by phone or online if you prefer. Call the forms and publications ordering line or visit the IRS site. Click on the "forms and publications" link, and choose the "forms and publications by U.S. mail" link from the list of ordering options.

    • 3). Indicate the year of the incorrect return on the front of form 1040-X. Fill out the form according to the 1040-X instructions specific to your tax situation, using columns A, B and C. These columns are for the original amount, net change and correct amount respectively. Attach all forms -- including W-2s -- and schedules that need to be corrected. Send them to the address appropriate for your tax situation. Look under "where to file" in the instructions for form 1040-X.

    • 4). Send your amended return to the Internal Revenue Service so it arrives within 3 years of the date of your original return, or two years from when you paid your tax bill -- whichever comes first -- if you are expecting a refund. Expect to pay penalties and interest on any amount due for amended returns not received within a year after the original return was filed.
